Required Graduate School Forms

Masters of Science Thesis Option (6 forms)

Each of these forms plays a different part in the completion and graduation process. Be sure to follow these directions. For additional information, review the Student Guide Document and/or contact the RS Department. These forms are a combination of online forms and PDF forms. Note, this section does not address IRB requirements, only UGA Graduate School forms and processes.

  • Deadlines: As you get closer to your graduation semester, familiarize yourself with the deadlines for the term in which you hope to graduate. Deadlines for graduation materials are found at grad.uga.edu/index.php/current-students/important-dates-deadlines/. In most cases, on the deadline page, you will also find links to the appropriate forms.
  • These Graduate School forms must be completed and turned in before the deadlines noted. If you turn in these forms AFTER the deadline, you can either plan on paying a late fee or postponing your graduation.
  • Do not wait until the last minute to submit these forms. Remember, all forms have deadlines. You MUST allow for collection of signatures and departmental processing times. If the deadline is June 3rd, then you should plan on having the forms into the department no later than June 1st.
  • Graduate students must register for a minimum of three hours of graduate credit during the semester in which degree requirements are completed. For RS Thesis students, this means you must enroll in PHRM 7300 the term you intend to graduate. Please use your full 9-digit CAN # (810 or 811) when filling out all forms.
  • If you have applied for specific semester graduation and find you will not be able to meet one of these deadlines, you must notify the Graduate School prior to the deadline, or your file may be placed on inactive status. Thesis FORMS (before your defense)

  • I. Apply for Graduation.  Follow the instructions on the Graduate School Forms page to login to Athena and apply for graduation This must be done ON, BEFORE, or WITHIN the FIRST TWO (2) WEEKS of the term you intend to graduate. You may apply one semester a head of time if you’d like. If you are unsure, go ahead and apply. Then, sometime within the current term when you have a better sense of your ability to complete the thesis, contact the UGA Graduate School at gradinfo@uga.edu advising them that you need to postpone your graduation.*
  • II. Program of Study form: Be sure to select the form on the Graduate School Forms page labeled Program of Study (MA and MS).  This must be done ON, BEFORE, or WITHIN the FIRST TWO (2) WEEKS of the term you intend to graduate. You may submit this form once you have completed 30-34 credit hours.Read these important points:

    1. Read the form instructions and follow the directions carefully.

    2. Be sure to include the * asterisk on the course boxes.

    3. Use the UGA Registration system to assist you in filling out course prefix, credit hours, terms, and grades received.

    4. For Degree, enter Masters of Science

    5. For Major, enter Pharmacy (if space permits you may include with an emphasis in Regulatory Affairs)

    6. No grade of incomplete can be included on this form. If you have an incomplete, address that incomplete before you submit your Program of Study form.

    7. Be sure to include your signature (digital signature is acceptable).

    8. Once you have completed the form to the best of your ability, you may submit this form to the Regulatory Sciences Department who will collect faculty signatures and submit this document to the UGA Graduate School.

    9. Fax or email your SIGNED COMPLETED form to the UGA RS Office. Fax number is 770-357-3806, or you may email it to the Assistant Director.

    10. Do not wait until the last minute to submit these forms. Remember, all forms have deadlines. You MUST allow for collection of signatures and departmental processing times. If the deadline is June 3rd, then you should plan on having the form into the department no later than June 1st.

    Use of Credit Hours

    Reminder about the use of your academic credit hours at the University of Georgia – Course and resident credit used to satisfy the requirements of one degree cannot be used to satisfy the requirements of another degree.   • III. Advisory Committee form: Be sure to select the form labeled Advisory Committee (MA and MS). This must be done ON, BEFORE, or WITHIN the FIRST TWO (2) WEEKS of the term you intend to graduate. You may submit this form once you have identified your major professor and committee members.

    a. No signatures are required for this document.

    b. Fill it out according to the directions

    c. Fax or email your completed form to the UGA RS Office. Fax number is 770-357-3806, or you may email it to the Assistant Director.

    d. Again, do not wait until the last minute to submit these forms. Remember, all forms have deadlines. You MUST allow for departmental processing times. If the deadline is June 3rd, then you should plan on having the form into the department no later than June 1st.

  • IV. Thesis Format Check: First, check this site, grad.uga.edu/index.php/current-students/important-dates-deadlines/, to find out the thesis format check deadline. Then, proceed to grad.uga.edu/index.php/current-students/policies-procedures/theses-dissertations-guidelines/format-check/. This link will provide you with detailed information on the conformity of your thesis. If your thesis document does not meet these standards, it could delay your graduation. If you feel you have satisfied all the requirements below, you may proceed to the format check submission page. You must have a format check done before you can submit your official copy.
  • V. Request for Services:  As you plan your thesis defense, you will likely need to request some departmental services, like room, teleconference, web-conference reservations or equipment. Once you have identified the date, time, and manner of delivery with your committee, contact the RS office directly to arrange for services.

Thesis FORMS (after your defense)

Generally, it is a good idea to fill out these forms ahead of the date of your defense and bring them with you to the meeting. If you are defending remotely, fill these out ahead of time and send to the Assistant Director. Remember, fill out only those form fields you know and submit to the department for faculty signatures.

  • VI. Thesis Defense & Final Examination Approval form: Be sure to select the form labeled Thesis Defense & Final Examination Approval (MA and MS). Once you have completed the form to the best of your ability, you may submit this form to the Regulatory Sciences Department who will collect faculty signatures and submit this document to the UGA Graduate School.
  • VII. Electronic Thesis & Dissertation (ETD) Submission Approval form: Be sure to select the form labeled Electronic Thesis & Dissertation (ETD) Submission Approval. Once you have completed the form to the best of your ability, you may submit this form to the Regulatory Sciences Department who will collect faculty signatures and submit this document to the UGA Graduate School.
